CBLO stands for CORA-Based Leveling Osteotomy. It is a surgical technique used to correct cranial cruciate ligament (dog ACL) disease and tears. Cruciate disease is one of the most common knee problems diagnosed in large-breed dogs. It can lead to significant arthritis, joint pain, and mobility problems, and negatively impact the quality of life. When used early in the course of chronic cruciate disease, it can stop progression and preserve the remaining cruciate ligament. It can also be used after a full cruciate tear to stabilize the knee and slow or prevent arthritis. This procedure involves cutting a portion of the tibia (shin bone), rotating it, and stabilizing it with a screw and bone plate.. This changes the angle of the top of the tibia that the femur (thigh bone) rests on to make it less steep so that the knee is more stable during weight bearing. The benefits of a TPLO over other surgical techniques include:
- Faster healing times (80% of patients are healed by 5 weeks post op)
- More secure stabilization with less risk of implant failure compared to extracapsular repair (lateral suture) techniques, especially in large, active dogs
- Ability to preserve remaining ligament fibers in patients with partial cruciate tears
- Can be performed on young dogs with open growth plates
- Can be performed concurrently with luxating patella correction surgery if indicated
- Can be performed to correct cruciate disease in dogs with excessive tibial slope
